Job Description:

GENERAL SUMMARY:  The PRN speech language pathologist (SLP) will provide comprehensive and skilled SLP services. The SLP will be responsible for program development, enhancing efficiencies and delivery of care, quality improvement, and compliance to regulatory guidelines. 
Interdisciplinary care plans:

  • Complete plans with short and long term goals that are medically necessary and measurable and are relevant to the patient’s needs.
  • Interventions are selected based on the client’s individual needs.
  • Information from patient’s evaluation, treatment and individual care plan is communicated in a timely manner to ensure the patient’s well-being.
  • Communicate to all necessary entities: doctors, nurses, patient, family and any other participant in the multidisciplinary team while ensuring HIPAA rules are maintained.
  • Complete screening, evaluations and follow up care on patients and residents as clinically and medically necessary per physician orders.
  • Adhere to policies and practice of applicable professional organizations and facility.
  • Coordinate treatment with other disciplines, participate in multi-disciplinary meetings, rehab meetings and facility wide meetings.

Document the course of treatment with progress made, barriers to progress and continuing need for services.  Documentation daily, progress notes, evaluations, updated POC, discharge summaries and all other relevant documentations per policy and procedures.
Participate in discharge planning. Participate in facility quality improvement and survey preparedness
Development:

  • Participate in continuing education and maintain current license/certification.
  • Assist with the development of student clinical programs and provide clinical oversight in accordance with ASHA, state practice Act and per Facility policy.

Provide patient, family and staff education as necessary with personalized methods for education to meet resident needs. 
Contribute to facility relations and marketing activities.
Work collaboratively with supervisor, coworkers, patients and families.
Perform other duties as assigned.

ESSENTIAL QUALIFICATIONS:

(Any Equivalent Combination of Knowledge, Skills, Abilities, Education and Experience)

 

  1. Christian Commitment: A deep and abiding faith that is fully surrendered to the will of Jesus Christ, evidenced by a lifestyle that is consistent with biblical principles in word and deed. Commitment to spiritual growth and development.  Demonstrates the values of a CRISTA leader.

 

  1. Education: Master’s degree in Speech Pathology/Communication Disorders (SLP) from an accredited college or university recognized by the United States Department of Education or equivalent.

 

  1. Licensure/Certification:
    • Current WA license to practice.
    • ASHA Certificate of Clinical Competence in process.

 

  1. Other Considerations: Subject to call-back during emergent or special situations. Healthcare provider required to comply with the federal law (HIPAA) that addresses privacy and security of protected health information. HIPAA training required. Ability to work collaboratively with supervisor, coworkers, patients and families.

 

  1. Software and Equipment Used: Outlook Express, Microsoft Excel and Word.

  1. Experience: Working as a SLP with patients/residents/clients in a related setting.

 

  1. Other Considerations: Must be able to communicate effectively with all levels of employees and residents in the SNF. Must be able to serve as a teacher and provide guidance towards best practices to staff.
